DOCUMENTS NEEDED FOR REGISTRATION
**Copy of Birth Certificate – MUST HAVE BEFORE COMPLETING AN APPLICATION.
**Proof of Address/Georgia Residency – MUST HAVE BEFORE COMPLETING AN APPLICATION. (For example: electric, gas, home phone, water bill, rent receipt, letter from employer if rent is provided, or mortgage payment – (Drivers license or a cell phone bill is NOT accepted).
**Copy of Social Security Card, Medicaid Card, Food Stamp Card
**Copy of Immunization Form 3231
